Piper Alpha was not destroyed because one form went missing. The disaster showed what happens when maintenance status, shift knowledge, production authority, fire protection, pipeline dependencies and emergency command cease to operate as one controlled system. The accountability question is therefore not who handled the last sheet of paper, but who could know the plant's true condition, stop hazardous work or restart, interrupt connected production, protect escape routes and prove that those controls worked before lives depended on them.
