The 1987 King's Cross Underground fire began as a small, survivable ignition beneath a wooden escalator and became a mass-fatality disaster in about 20 minutes. The public inquiry found not one inexplicable failure but a connected control breakdown: combustible and contaminated escalator machinery, weak maintenance learning, no automatic detection, delayed escalation, an unpractised evacuation system, and emergency services operating without a common command picture. This analysis reconstructs that sequence before assigning accountability, separates Sir Desmond Fennell's safety findings from legal adjudication, and tests whether later engineering, regulatory and management reforms created durable evidence of safer control.
