Chernobyl Nuclear Power Plant

The destruction of Unit 4 at the Chernobyl Nuclear Power Plant was not only a failed turbine rundown test or a sequence of operator errors. It was the exposure of an assurance system that allowed hazardous reactor characteristics to remain poorly disclosed, made compliance difficult to see in real time, tolerated weak independent challenge, and then forced workers, communities and neighbouring states to depend on institutions whose emergency information was incomplete. Accountability therefore runs through design authority, operating discipline, regulatory independence, public warning, exposure evidence, remedy and proof that later reforms work.
