The Hyatt Regency walkway collapse was not merely a story about one altered steel detail. It was an accountability failure in which a consequential connection change moved through design, detailing, approval, fabrication, construction, inspection, and occupancy without a complete structural check being made visible. The technical mechanism is now well established. The harder question is institutional: who had practical control over the change, who was professionally responsible for verifying it, what signals should have stopped the work, and what evidence would show that later controls actually became stronger?
